Custom Ticket submission fields

How do I add custom fields to the ticket submission form?  We need password info for certain email fixes or to unlock screen savers when users aren't around.  We're working on migrating to the built in system away from Spiceworks.

In the queue configuration there are options for setting up the custom fields. Set the permissions on the field to allow the user to modify it and the field should appear on the ticket submission form.

I personally would not recommend asking users to submit password information with a ticket, however. That data will be stored in plain text in the KACE database.
